Thrombocytopenias Blood platelet levels normally fall within the broad range of 150400 109 L, although for any given person the range is much narrower.26 These levels are preserved by the fine balance between synthesis of platelets in the bone marrow and their removal by the spleen. Disturbing this balance by reducing production or increasing removal either by splenic uptake or platelet activation ; results in thrombocytopenia, which may be considered mild 100 150 109 L ; , moderate 50100 109 L ; or severe 50 109 L ; .26 In general, thrombocytopenias fall into 2 categories: inherited Table 4 ; 26, 27 and acquired Table 5 ; .26 Inherited thrombocytopenias are rare, are often associated with alterations in platelet size and, in many cases, are considered to be 1 component of a syndrome.27 Acquired thrombocytopenias can be classified as those of immune origin and those of nonimmune origin. Of the immune causes, immune thrombocytopenic purpura ITP ; is likely the most common and can occur in association with infection or as part of an auto-immune syndrome. Nonimmune acquired thrombocytopenias are most often secondary to drug toxicity or another underlying disease. 26 In particular, most chemotherapeutic agents can cause thrombocytopenia, and aggressive multi827e and zithromax.

The Crisis Intervention Team introduced by the Trust one year ago has been hailed as a major success. Over the past twelve months, the Team has succeeded in reducing emergency admissions to hospital for people experiencing a mental health crisis by 32%. The Team has received 2, 500 referrals of whom about 15% were admitted to hospital. The Crisis Team treated 800 people intensively at home to avoid admission to hospital. "We provide a viable alternative to admission to mental hospital, " said Mark Deveney, manager of the crisis resolution and home treatment service. "Doctors, nurses, occupational therapists, pharmacists and social workers are available to visit people at home when they need help most. "It is much better for somebody to be cared for at home rather than in a hospital environment. People recover more swiftly from both mental and physical health problems in their own home, where surroundings are familiar and they feel more at ease. Also, freeing up hospital beds reduces pressure on the system and means that clinicians can spend more time caring for those who require hospital admission." The crisis intervention team is one of only eight such services in England and Wales that operate 24 hours a day. It is the biggest in the country in terms of population cover and catchment area. The 33 members of the team cover 500, 000 people across a 6, 000 square mile area. The service they provide operates 24 hours a day, 365 days a year. "Most of the other teams in the country operate in urban areas, " said Mark. "As we serve a rural community, we are faced with specific challenges. We have solved the problem of communication by equipping everyone in the team with a laptop computer. This enables colleagues to keep in touch and record information about patient treatment when they are out of the office. Everyone involved with a patient's care is able to immediately access the latest details about their condition, medication and so on." The groundbreaking nature of the initiative has been recognised, and members of the team have been invited to make presentations to colleagues in Belgium and Ireland, as well as other parts of the UK. "We have had a lot of interest from professionals in a variety of areas, " said Mark. "We are happy to share our experience with other colleagues who are looking to set up a similar service." Referrals to the team come via GPs, Accident & Emergency, the police and occasionally night shelters. The target for the team is to visit the person within four hours of receiving the referral. At the visit they make an assessment of the individual's needs, which may or may not include hospital admission. With the patient's consent, their family are informed of all treatment plans. The crisis intervention team takes a holistic approach to patient care. They involve themselves in every aspect of the patient's needs, such as housing, benefits, and even whether a pet dog needs to go into kennels while the patient recovers. "Our philosophy is that if someone has a mental illness, then everything that contributes to their recovery is our business, " explains Mark. The service also provides a 24-hour telephone line providing advice and support to anybody already engaged with mental health services and zoloft.

Benefit in a recent PhIII breast cancer trial, ALTH has re-submitted RSR13 data from a substantially sized patient subgroup in which the drug has shown statistically significant clinical activity. MAXM's Ceplene histamine ; is in PhIII for melanoma with encouraging data and the company has over $100m in cash 3Q03 ; . The risk here clearly lies in the "out of the box" thinking around the use of histamines. However data will speak louder than words. Finally, although being tiny, XTL is worth a second M&A look: acquiring this company brings with it two fully human HepB and HepC antibodies and the Trimera mouse enables R&D using human immune system and human tissue in a mouse ; . PhI data of the antibodies in liver transplant patients looks encouraging and PhII trials are ongoing. Interested buyers will likely reside in the transplantation and or hepatitis space.

Extended. If you are traveling by plane, pack your medications in a carry-on bag. Check with your airline to see if there are any restrictions in taking your medications on the plane. Taking extra medications and packing them in a different bag may also be helpful should you or the airline misplace your luggage. Take your important phone numbers with you in case you need to call your transplant center while you are away. You may want to drink bottled water if you are traveling in areas where the sanitation is of concern. Most transplant centers discourage travel to third world countries for patients who are immunosuppressed. The risk of acquiring infections in these areas is high and appropriate healthcare may not be available. Additionally, some foreign travel may require immunizations that are not safe for patients who are immunosuppressed, such as measles and smallpox. If you plan to travel in this type of area, please discuss this with your transplant doctor and coordinator so that you are fully informed of the risk. It would also be helpful to learn more about the health concerns in your desired area of travel through the Travelers' Health website sponsored by the Centers for Disease Control at : cdc. gov travel destinat This site discusses current illnesses and diseases occurring in that region, required vaccinations, health risks, and how to stay healthy during your visit. How an Unwanted Medication Collection & Destruction Project Would Operate in New Hampshire BACKGROUND The Northeast Recycling Council, Inc. NERC ; , a non-profit organization that works on issues related to solid waste, recycling, and decreased toxicity of the solid waste stream, has been awarded a grant by the U.S. Department of Agriculture to develop environmentally sound and legal strategies for collecting and destroying unwanted medications from consumers. The need for such a project arose from the growing scientific evidence that over-thecounter and prescription medications are finding their way into water supplies. While non-metabolized medication is certainly the primary source of this contamination, disposing of unwanted medications down the drain or in the trash has added to this problem. This project has sought to develop and test strategies for preventing unwanted medications from environmentally unsound disposal, while ensuring that they are destroyed and protected from diversion. A component of the grants is to develop and test collection strategies throughout the multi-state region. All collections are supervised by law enforcement and all collected medications are destroyed. In 2005, collections were held in Maine and Massachusetts, with the hope of holding events in Connecticut, New Hampshire, New Jersey, and Vermont in 2006. Any collection event held in New Hampshire will fully implement all best management practices and legal requirements that have been identified and developed through these grants. The development of collection strategies has been the result of the collaborative efforts of many individuals and organizations, including and acomplia.

Immunohistochemistry of Cell Clot Paraffin Sections: An Efficient and Accurate Method for the Detection of Intracellular Antigens, Including Prognostic Markers, in CLL. Alexandros D. Polydorides, * Elizabeth M. Hyjek, April Chiu, Scott Ely, Wayne Tam, Susan Mathew, Daniel M. Knowles, and Amy Chadburn. Sponsor: Ellinor I.B. Peerschke. Department of Pathology and Laboratory Medicine, Weill Medical College of Cornell University, New York, NY. Chronic lymphocytic leukemia CLL ; , usually diagnosed by flow cytometry FC ; of peripheral blood lymphocytes PBLs ; , is prognostically divisible based on immunoglobulin Ig ; gene status unmutated-poor; mutated-good ; . CD38 and ZAP70 antigen expression in CLL specimens correlates with unmutated Ig. CD38 is easily detected by FC, but ZAP70 and MUM1, a putative prognostic antigen, are intracellular and best detected by immunohistochemical staining of tissue specimens. We describe a method of generating cell clots CCs ; in a thrombin-fibrin matrix developed by E. Hyjek ; that reliably detects intracellular antigens. CCs were generated from cell suspensions of PBLs from 42 CLL, 16 acute leukemia, 4 mantle cell lymphoma, and 3 non-CLL patients and normal controls, fixed in formalin, and routinely processed. Paraffin sections from each CC were stained with H&E.

Be evaluated on an equivalent basis to synthetic compound libraries. The disadvantages are that it is a time-consuming process and minor active components may be missed using an isolation strategy based solely on peak collection. Dereplication. The term "dereplication" is the process of identifying known compounds that are responsible for the activity of an extract before bioassay-guided isolation has started.99, 118-121 The dereplication process, which has been an ongoing concern in NP chemistry since the beginning of antibiotic research, is used to eliminate, group, and or prioritize extracts for further study and can save considerable research time. The dereplication procedure also can be extended to group like-extracts that contain the same or similar unidentified compounds that are responsible for the biological activity. This grouping of extracts with like dereplication profiles significantly reduces the possibility of different chemists independently isolating and identifying the same active component. The most generic procedure used today involves separation of an extract using reversed-phase HPLC and splitting of the eluent postcolumn into a mass spectrometer usually MS MS to match fragmentation ions ; and a fraction collector using microtiter plates to test tubes depending on the scale. The fractions are screened and the retention time, UV spectra, MS data, and activity of fractions are analyzed for common interfering compounds and known inhibitors using commercial and in-house databases. This method has been so successful that it has also been used in combinatorial chemistry to analyze reaction mixtures.122 Other methods for dereplication that have been used include separation by solid-phase extraction cartridges121 and CCC.115 Alternative methods for analysis include affinity capillary electrophoresis, 123 LC-NMR, 124-126 and online biochemical assay.108 Isolation and Structure Elucidation. The bioassayguided fractionation procedure used to identify bioactive natural products is often perceived as rate limiting and resource intensive. However, the rapid improvement of instrumentation and robotics used to revolutionize other aspects of drug discovery can also be used to improve the speed of the isolation and structure elucidation of NPs. After dereplication, extracts then undergo bioassay-guided fractionation to ultimately provide the active compound or compounds. An increase in the speed of bioassay-guided fractionation has been facilitated by a marked improvement in HPLC automation, MS, and column technology, as well a rapid turnaround of screening results provided by HTS. The advent of new probe technology127 and higher magnetic fields has led to a significant shortening in acquisition time for NMR data, and the structure elucidation of NPs can be achieved routinely on amounts less than 1 mg.128 Progress has also been made in automated structuresolving algorithms, 129-131 but presently none can rival the structure elucidation skills of an experienced NP chemist. However, these programs can be a valuable tool to search for alternative structures that fit the same NMR data. Compound Development. Perhaps the biggest obstacle to NP chemistry is the continual resupply of large amounts of NP required for further biological evaluation. The identification of a sustainable source of the NP needs to be addressed if a semisynthesis or total synthesis is not available. This is not so much trouble if there is a microbial source of the compound, which explains why most companies prefer the screening of microorganism extracts. A systematic approach called OSMAC one strain-many compounds ; to increasing the yield and diversity of com.
