The recent advances in antimicrobial therapy, including the new macrolides, fluoroquinolones, and oxazolidinones linezolid ; , have improved the therapeutic options for the clinician and the prognosis of disease due to these organisms for the patient. This is especially true for M. chelonae and the M. fortuitum group. There is still, however, an urgent and compelling need for the development of better, more effective, and safe oral antimicrobials for the treatment of disease caused by the RGM, especially M. abscessus. M. abscessus lung disease, for example, is still generally incurable with currently available drug therapy. Susceptibility testing of the RGM is essential to choose optimal effective drug therapy and to monitor for the development of mutational drug resistance, which may occur with prolonged therapy. NCCLS tentative standards for such testing by broth microdilutions were published for the first time in December 2000. Taxonomically, the pathogenic RGM have undergone dramatic changes in the past few years. Multiple new species such as M. goodii, M. immunogenum, and M. houstonense proposed ; have been introduced, and some former subspecies or subgroups have attained species status. These taxonomic advances have been attributed primarily to the use of HPLC and the new molecular techniques such as PRA, ribotyping, hybridization, and 16S RNA gene sequence analysis. These molecular methods, especially PRA, are rapidly replacing conventional biochemical testing in the large reference laboratory. Identification of the RGM by these molecular diagnostic methods may not only improve the correct recognition of current RGM species as well as identity previously uncharacterized species, but will also decrease the traditional laboratory delays in species identification and hence will lead to a more rapid and accurate diagnosis of disease. When was the last time you saw a case of measles? If you're in a developed country and are under 40, the answer may be never. But that might be about to change, provided of course that you recognise what you see. The number of cases is increasing steeply in the UK, bringing predictions of the re-emergence of endemic measles. Meanwhile, as Perviz Asaria and Eithne MacMahon point out p 890 ; , clinical experience of measles has declined. They offer a reminder of the clinical features, complications, and management strategies. They also call for increased measles vaccine coverage if the UK is to meet the World Health Organization's target of elimination by 2010. UK vaccination rates remain below those achieved before Andrew Wakefield published his Lancet paper on MMR in 1998, and efforts to regain lost ground have been only partially successful. In the battle for hearts and minds, the media and anti-vaccine groups have mounted a far more compelling case than the Department of Health. In view of this, plans to pursue Wakefield for misconduct through the General Medical Council seem doomed and dangerous. Doomed if the main charge is publishing flawed research because that would set an impossible precedent. So much research is flawed, the GMC would be overwhelmed. Dangerous because, even if successful, the case would refuel the controversy and present Wakefield's supporters with a platform. Part of the problem is the perception that no one in an official position has taken seriously the concerns of families who believe their children have been damaged by the vaccine. The denial that the vaccine has caused the damage, though almost certainly accurate, doesn't sound very sympathetic and leaves Wakefield with a monopoly on taking these concerns seriously. Some say that the Wakefield saga may have helped medical science by making us all more careful about the research we perform and publish. I note that two of the research papers in this week's BMJ are randomised trials, the other being a well performed qualitative study. ; But it would be bad if we became too cautious, especially if journals shied away from controversy. I see no sign of that. As if to prove the point, we publish this week a broadside based on a systematic review of the literature ; about the lack of evidence for influenza vaccine. Why, asks Tom Jefferson p 912 ; , is there such a gap between evidence and policy? Governments go to great lengths to promote and provide the vaccine. But there is almost no valid evidence that it does any good. Jefferson puts the gap down to our desire to do something, combined with "optimism bias"--an unwarranted belief in the value of interventions. Would randomised trials be unethical? No, says Jefferson, they are the only ethical response to the possible waste of resources on ineffective or only partially effective care. The problem is that the UK has no transparent process for evaluating the effectiveness or cost effectiveness of vaccines. NICE would like to take this on. The government should let it. Overactive Letdown If your baby triggers your milk ejection reflex and then pulls off, seems unable to keep up with the flow, or makes gulping or clicking noises it's possible that your letdown is too strong for your baby. When babies try to keep up with strong flow they tend to swallow air which contributes to general gas and Reflux. Try pulling baby off and spraying into a towel until the spray slows, pumping or hand expressing until your spray slows a little, or nursing while reclining with baby lying on top and facing you. Feeding on one side per feeding will help manage the number of letdowns your baby has to deal with as well as any oversupply problems. Smaller, More Frequent Feedings Smaller feedings are more quickly digested and less likely to come back up. Also a too-full tummy can put pressure on the lower esophagus sphincter, causing Baby to spit up. Your baby may want to eat more often. Frequent Burps Breastfed babies do not require as much burping because they don't swallow as much air during feeds. You may want to try burping between breasts, anytime your baby pulls off your nipple, or gets squirmy. Bottle fed babies should be burped frequently, even as often as every 1-2 ounces if they seem irritable or gassy after a feed. If your baby gets too upset when you remove the bottle it may be best to wait until after 3-4 ounces initially before attempting to burp. Most babies will get upset but will feel better after a chance to burp. Block Nursing If your baby seems to need to nurse all the time, has frequent diarrhea or green stools, seems excessively gassy or pulls off at the beginning of feeds, or if you seem to leak large amounts of milk between feedings it could be that you have a foremilk hindmilk imbalance. This is generally caused or aggravated by certain nursing habits or oversupply. It is easily correctible by altering nursing habits. Your breastmilk changes in composition throughout a feeding. Early in the feeding your baby receives your foremilk. Foremilk looks much like skim milk, it is thin and watery and may have a bluish tinge to it. It is very thirst quenching, high in volume but low in calories and fat. Later in the feeding your baby receives your hindmilk. It resembles whole cream, low in volume but high in calories and fat. In a two to two and a half hour period try feeding your baby from only one breast no matter how many times he wants to feed. This will help ensure that he has totally emptied your breast and gotten the right amount of hind milk. This will help provide him with the same number of calories that he was getting with a smaller volume of food in his tummy and he may need to feed less often.

This subchapter implements Chapter 28 of the code relating to the Electricity Generation Customer Choice and Competition Act ; governing adjustments to the rates of an EDC to reflect changes in its State tax liability.
