Yerevan State Medical University After Mkhitar Heratsi, Yerevan, Armenia The goal of our study was the revealing of molecular biological mechanisms of phospholipids qualitative and quantitative changes in hepatocytes chromatin after bilateral sub-diaphragmatical vagotomy, solarectomy removal of plexus solaris ; and their combination. Data obtained show considerable changes in content of total and individual phospholipids, particularly of phosphatidylcholines, phosphatidylethanolamines and polyphosphoinositides that testify expressed destroys in functioning of phosphatidylcholinic and phosphoinositidic cycles. The changes of ratio of some neutral phospholipids to some of acidic also take place. It can be a cause of changes of structural organization and functional activity of membranes and sub cellular formations. Above-mentioned disorders are more expressed after solarectomy and its combination with vagotomy. These investigations once more indicate the significant role of Vegetative Nervous System and its peripheral parts in regulation of subcellular structures and cells functioning. There also were the changes of hepatocytes mitochondrial fractions' phospholipids. The results of related investigations can be as a new qualitative step in confirming of L. A. Orbeli's biological conception reality about the adaptational-trophical role of Autonomic Nervous System on subcellular structures and cells in whole, for example, generic name.

Stroke is one of the most preventable of all life-threatening health problems. Risk factors for stroke fall into two categories: those that can be controlled through lifestyle changes or medication and those that cannot. It's important to remember that having one or more uncontrollable stroke risk factors DOES NOT MAKE A PERSON FATED TO HAVE A STROKE. With proper attention to controllable stroke risk factors, the impact of uncontrollable factors can be greatly reduced and ketorolac. R. G. JACOB instructed to increase finger temperature during relaxation sessions [c.f. 5 ; ]. A "self control model, " was used as a rationale for the biofeedback treatment. For example, paradoxical decreases in temperature often occurred during initial sessions in which feedback was provided to increase temperature. These paradoxical decreases represented an opportunity to teach the patient about the adverse effects of "trying too hard" and the importance of "passive volition." If the patient's finger temperature already was high at the beginning of a period, instructions to decrease temperature were provided. The mental imagery used during feedback to decrease instructions often involved "stressful" scenes; patients were thus made aware of the adverse effect of "stress" on finger temperature and by implication, blood pressure ; , instructions to increase or decrease were given during each of the three periods baseline, feedback, generalization ; of the session. Overall, 67% of the periods involved increase instructions and 33% involved decrease instructions. Each patient was exposed to increase instructions. One patient who was particularly successful with temperature control progressed to feedback of foot temperature. The stress education program was designed to incorporate the subject matter typically covered in stress management programs. The content, however, was covered only in general terms without recommendations for actual behavioral change. Furthermore, all references to relaxation or meditation were omitted. As home practice, subjects self-monitored events and symptoms of stress and completed various questionnaires relevant for the program. During the next session, the therapist spent 5 to 15 minutes listening to the patient discuss stressful events and providing general support by asking further questions; she then proceeded to the content planned for that session, including such topics as the effect of stress on blood pressure, coping styles, time management, and problem solving. At all times, the therapist refrained from giving direct advice. Both treatments were conducted by the same therapist, a psychiatric nurse with additional qualifications in exercise physiology. To enhance compliance with the medication regimen, the importance of taking the prescribed medication was emphasized during both the Hypertension Clinic visits and the sessions with the behavioral therapist. Furthermore, the patients performed daily monitoring of their prescribed medication using self-monitoring sheets. These were reviewed by the behavioral therapist at each session. In case of noncompliance, the therapist was instructed to 90 intervene with a problem-identifying assessment and problem solving behavioral counseling. Such interventions were seldom needed. Campaigners yesterday said the european medicines agency was reviewing data on the potential cardiovascular effects of cox-2 inhibitors and ketotifen, because side affects.
Nasogastric tubes usually range from 8 to 12 French in diameter and 30 to 36 inches in length. Small-caliber nasogastric tubes are used solely for feeding, whereas largecaliber tubes can be used to decompress the stomach, monitor gastric pH, and deliver medications and feedings. Large-caliber nasogastric tubes are usually made of stiffer.

This white paper used a broad, third party definition of SDP -- one that reflects the fact that service delivery is an end-to-end process. In turn, this indicates the need for an end-to-end architecture service environment. There is also a clear and compelling need to ensure that the SDP meets all business and technical goals, both now and in the future, i.e. it must be based on standards and open APIs. The SDP decision can not be made from product perspective only. The starting point has to be the business objectives: the SDP must map to the operator's strategy; it must not determine that strategy. We therefore developed a well-defined evaluation and definition process. It starts, as this paper has shown, with the value chain, a concept that is used to position the key SDP domains within that end-to-end architecture. This makes it easier to establish a clear understanding of the individual roles and functions of each domain and how that functionality should be distributed and optimized. The service delivery framework provides a logical representation of the whole service delivery environment. This is done in order to establish an outline agreement on the key business criteria. The reference architecture is then used to define a physical framework and establishes a common language and a common understanding between all parties involved during the consulting phase. Once established, it is easier to reach agreement on identifying the key components and functions required to implement a service delivery platform. Thus, the framework is actually a reference model used to define a customized instance and its implementation. Products are introduced at the end of the process and functionality can now be assessed on the basis of the business goals that were defined and agreed at an earlier stage.

It all starts with brain chemistry, " says Dr. Braverman. "When you start to lose your edge, look up your physical symptom and restore the neurotransmitter that is responsible for it." Brain chemistry is difficult to measure directly. But brain electrical activity, which results from chemical reactions, is now easy to assess with Quantitative Electroencephalography QEEG ; , also called brain mapping5, 6. Just as an EKG measures electrical activity in the heart, similar electrodes are placed on the scalp and Dr. Braverman uses a computer with special interfaces and software to record and interpret the four key electrical measures of brain health: voltage or energy ; , latency or speed ; , rhythm or balance ; , and synchrony or symmetry ; . These measurements reveal the core of what we need to know about dopamine, acetylcholine, GABA, and serotonin levels, respectively7. Brain chemical imbalances are uncovered by specific brainwave abnormalities. But you don't need a sophisticated test to maintain health. Because illnesses are associated with.

M. canis is probably related to the increase in the number of domestic animals particularly cats living outside of homes and consequently an increase in the phenomenon of animals stray and semistray. Presumably stray cats are the most important carriers and transmitters of M nis. Our findings indicate constantly increase of zoophilic dermatophytes, especially M. canis. Like in many countries in southern Europe, in Bosnia and Herzegovina as well, antropophilic dermatophytes were replaced by zoophilic species. ISE.322 Common Pediatric Dermatoses: An Epidemilogical Study E. Kasumagic-Halilovic, M. Kantor, A. Prohic, F. Alendar. Department of Dermatology, University Clinical center, Sarajevo, Bosnia and Herzegovina Dermatomycoses account for the majority of skin diseases and are of particular interest in pediatric patients as fungal elements can be easily spread from one carrier to the next. This study was conducted to determine the prevalence of skin mycoses and their causative agents among children under 14 years. Samples were collected from 1225 children attending the Department of Dermatology in Sarajevo during the period 2000-2005 and were submitted to a mycology laboratory for analysis. Fungus elements were demonstrated by microscopic examination in 873 71.3% ; cases and the etiology was determined by culture in 699 80.1% ; cases. Dermatophytes were demonstrated in 251 35.9% ; , candidiasis in 375 53.6 ; and pityriasis versicolor in 73 10.4% ; samples. The most common dermatophyte infection included tinea capitis 112; 44.6% ; and tinea corporis 56; 22.3% ; . The most prevalent species were Microsporum canis 221; 88.0% ; and Trichophyton mentagrophytes 22; 8.7% ; . Candidiasis was observed the most often in infants in the first year of their life 250; 66.6 ; , in groin region as diaper dermatitis. The most common species was Candida albicans 297; 79.2 ; . Pityriasis versicolor was diagnosed in 73 children, 91% between 10 and 14 years. Among seven different isolated species, Malassezia globosa in its mycelial phase was found to be the most frequent species, as well as in adults. ISE.323 Fulminant Fungal Peritonitis and Ascites in A HIV-infected Patient with HCV-related Chronic Hepatitis. A Role for Prolonged Nimesulide Self-administration? R. Manfredi, S. Sabbatani, F. Chiodo. Infectious Diseases, University of Bologna, Bologna, Italy Background: The mortality rate of HIV-infected patients p ; with liver disease is substantially increasing. Methods: An exceedingly rare case of Candida albicans fulminant peritonitis and ascites in a p with HIV-HCV-coinfection and stable cirrhosis, and possibly related to exaggerated self-administered nimesulide, is reported. Results: A 46-year-old p with HIV infection known since 14 years received isolated 3TC-d4T since six years with a favorable laboratory response: HIV-RNA 480 copies mL, CD4 + lymphocyte count 428 cells L. Neither liver biopsy nor specific treatment were performed for a concurrent stable HCV liver cirrhosis. Two months before admission, our p had a shoulder fracture, and uncontrolled nimesulide self-medication was performed during six weeks. A rapidly worsening ascites and oliguria led to admission. Slightly increased ALT, amylase, and bilirubin were detected, but a rapidly increasing ascites and diffuse edema occurred, paracentesis, and diuretic-albumin administration failed, and the worsening ascites-anuria evolved into kidney failure. One day later our p deceased and necropsy examination showed a diffuse polyvisceritis and a micronodular hepatitis with abundant ascites, in absence of kidney-urinary tract anomalies and other signs of decompensated cirrhosis. After p's death multiple ascites cultures yielded isolated Candida albicans. Conclusions: HIV-infected p have increased risks of liver toxicity. NSAID are implicated in severe, and possibly lethal hepatotoxicity. The exceedingly rapid and severe evolution towards a Candida-infected ascites associated with refractory anuria, in absence of decompensated cirrhosis, acute hepatotoxicity, and kidney involvement at autopsy, was never observed after NSAID nimesulide use. Animal models showed a NSAIDinduced increased enteric vascular permeability causing infectious peritonitis. Clinicians facing p with advanced chronic hepatitis but no decompensated cirrhosis, should remind that NSAID may act on liver-bowel function, and could prompt a liver-kidney damage, possibly complicated with infectious ascites, for instance, monographs.

Critical Performance Numbers for Pilots Part Two Weight and Body Mass Index This is the third in a running series of articles on critical numbers for pilot health. Previous articles covered blood pressure and cholesterol. They are available on our web site at AviationMedicine under the VFS News section. Just as an aircraft performs differently with varying weights and balances, so does the human body. Not only does total body weight have an influence on health, but the distribution of fat on the body also may predict risk for cardiac disease. Obesity is an epidemic in the United States. Obesity is defined as a weight greater than 30% of the ideal body weight. Overweight people have weights between 20% and 30% greater than ideal body weight. In the US adult male population, 32.9% meet criteria for obesity and an additional 25% are overweight. In US women, the percentages are 25% and 25% respectively. Disturbingly, over 18% of US children are overweight with a rapidly rising percentage. Obesity is more prevalent in Hispanic and non-Hispanic Non-white populations. More recently, the criterion for obesity is defined by Body Mass Index BMI ; . This number incorporates the ratio of a person's height and weight allowing the use of uniform numbers for persons of all heights to define criteria for weight categories. The formula for BMI is weight kg ; [height m ; ]2 or weight lb ; [height in ; ]2 x 703 BMI BMI tables are available on a variety of web sites, including the Centers for Disease Control at : cdc.gov nccdphp dnpa bmi . General categories include: Underweight Normal Overweight Obese Morbidly Obese BMI BMI BMI BMI BMI 18.5 and 24.9 25 and 29.9 30 40 Distribution of body fat also seems to have some predictive value for increased risk of heart disease. Men with large amounts of abdominal fat "beer bellies" ; are at higher risk for heart disease. Women who have body fat distributed primarily around the hips, the so-called "pear-shape", also seem to be at increased risk for heart disease. The reason for this finding is unknown. Although many factors may influence weight genetics, diseases, medications, etc. ; , the primary influences on weight are caloric intake and physical activity. The average middle aged male with little physical activity requires 2500 calories per day to hold weight stable, while the average middle aged female will burn approximately 2000 calories per day with little activity. If a person's intake of calories equals the daily output, weight will remain stable. Reducing caloric intake to less that daily output will result in a gradual weight reduction. Obviously weight gain occurs when more calories are consumed over time than are burned with the basic metabolic rate plus physical activity. Different food types have different caloric values. A gram of protein contains approximately 4 calories. The same is true for a gram of carbohydrates simple or complex carbohydrates both have 4 calories per gram ; . Fats contain 9 calories per gram. Plain water has no calories. One ounce of food weighs approximately 28.5 grams, often rounded to 30 grams per ounce for convenient calculations. A pound equals 16 ounces or 454 grams. Using the above figures, a pound of body fat has the energy equivalent of about 4000 calories. Therefore, to lose a pound of body fat per week, a person would need to use 570 calories a day more than the daily intake of calories. 570 calories x 7 days 3990 calories per week ; . Most reputable and healthy diet plans recommend limiting weight reduction to one pound or less per week sustained over an extended period. Rapid apparent losses in weight after a period of exercise usually reflect loss of water in the body and are quickly regained with hydration. Diets that still provide adequate nutrients for health will contain at least 1200 1500 calories per day. Therefore, the optimum approach to weight reduction combines a modest reduction in average daily caloric intake with nearly daily exercise. The exercise does not have to be vigorous for successful weight reduction; even 30 minutes of walking per day broken up into several short intervals will increase caloric output. Since 1997 the American and British authorities in diabetes have been promoting aspirin prophylaxis against cardiovascular events in high risk patients with diabetes. However, only a small minority of them have actually been given aspirin. Only around 20% of patients with diabetes were found to be taking aspirin in the most recent United States National Ambulatory Medical Care NAMC ; survey.
Subjects Between 1995 and 1998, patients with a specific diagnosis of schizophreniform disorder or schizophrenia diagnosed by DSM-IV criteria, diagnosis should be equal at first admission and at end of study ; were included in our study and gave informed consent prior to participation in the study. We did not include patients with schizoaffective disorder, since this indication has a different treatment guideline with other considerations for psychotropic comedication use especially for antidepressant use ; . Patients belonged to a naturalistic cohort of 138 registered incident firstpsychosis patients in the northern part of the Netherlands. Incident first-psychosis was defined as being not diagnosed with psychosis at an earlier stage anywhere else in the northern region of the Netherlands. Subjects were recruited between 1995 and 1998 in a psychiatric unit of a university hospital in the northern Netherlands. During this period, no significant changes in treatment strategies were implemented in the psychiatric unit. The follow-up period of the study was 2 years. All patients who were admitted with first-break psychosis to an inpatient ward, were treated by an equal group of caretakers and eventually were referred to outpatient services. If referred, prescription of psychotropic drugs still occurred at the psychiatric unit, which enabled the follow-up survey of drug prescriptions. Sixty-four patients of the total naturalistic cohort could not be included because of other diagnosis than schizophrenia or schizophreniform disorder 46.4% ; . Six males and one female with a diagnosis in the schizophrenia spectrum were excluded, because of less than 3 months use of antipsychotic medication, change of diagnosis, refusal of medication, prematurely exit of hospital or transfer to a foreign psychiatric clinic. Data collection Information about diagnosis and treatment was extracted from care-as-usual ; medical records. We obtained data on type and dose of antipsychotic medication and concurrent psychotropic comedication. Dosages of antipsychotic drugs were compared by using the prescribed amount of the Defined Daily Dosage of the drug. For each drug, the WHO has defined a standard dose for the most important.
