Use of medication for treatment does not obviate the need to follow the recommendations for prevention of osteoporosis. A diet rich in calcium and vitamin D, as well as regular exercise, should be viewed as an integral part of any treatment regimen. The good news about osteoporosis is today, we know that low bone mass does not have to be a consequence of aging when proper prevention guidelines are followed. We can readily identify those individuals at greatest risk of osteoporosis, and reliable methods of early detection and treatment are available. However, osteoporosis continues to take a heavy toll nationally and in Florida, both economically and in a decreased quality of life for individuals suffering from osteoporosis. In the School Meet one-on-one with the principal and other administrators in your school to ensure that they are informed about ECPs. Discuss your ideas about increasing ECP awareness among staff and students. By contrast, the Baylor Breast Center 13 ; evaluated 177 patients identified from a tissue bank for whom ER and PR status were available for both the initial and the contralateral breast cancer. Of 114 women who did not receive any adjuvant systemic therapy, more than 80% of the women developed an ER-positive contralateral breast cancer regardless of the ER status of the first primary, suggesting that there was no correlation between the ER status of the original tumor and that of the contralateral breast cancer in the absence of therapy 13 ; . In women who received tamoxifen with or without chemotherapy, 50% of the contralateral breast cancers were ER-positive and 50% were ER-negative. By comparison with the study by Swain et al. 1 ; , the Baylor Breast Center study 13 ; has the advantage of including women who received no adjuvant therapy, affording us a snapshot of the natural history of the cancer. However, the Baylor Breast Center study 13 ; has the disadvantage of using a retrospectively identified population with uncertain biases. It is also notable that the median age at diagnosis of primary breast cancer in the Baylor Breast Center study 13 ; was 62 years compared with a median age at diagnosis of 48 years in the study by Swain et al. 1 ; . These findings suggest that selective estrogen receptor modulators can interfere with breast cancer initiation and or progression. Emerging data suggest that aromatase inhibitors administered instead of or after tamoxifen are associated with a substantial reduction in the incidence of contralateral breast cancer 16, 17 ; . It will be of interest to evaluate the hormone receptor status of contralateral breast cancers that develop in women taking aromatase inhibitors. In addition, several treatment trials 18 20 ; indirectly support the hypothesis that specific characteristics of the first breast cancer predict features of the contralateral breast cancer. Tamoxifen has been used as an adjuvant therapy for breast cancer for several decades and is now used for breast cancer prevention. Tamoxifen is not suitable as a substitute for estrogen in the prevention or treatment of osteoporosis, since it produces estrogenic effects on the uterus, including endometrial hyperplasia and endometrial cancer. It also has undesirable anti-estrogenic effects on the brain, resulting in hot flashes. Calcitonin Calcitonin is a 32-amino acid, single-chain peptide secretory product of neuro-endocrine C-cells located in the human thyroid gland. Calcitonin is a potent inhibitor of osteoclast function. There are two approved dosage forms, injectable which is rarely used and is associated with nausea, vomiting, and flushing ; and a nasal spray. After administration of calcitonin, there is a rapid decrease in bone resorption and hypocalcemia. However, good evidence of antifracture efficacy is difficult to find. Evidence from the Prevent Recurrence of Osteoporosis Fractures PROOF ; study of nasal calcitonin is compromised by the fact that only 378 women completed the study out of 1255 enrolled.17 In this study, a 400-unit dose of calcitonin nasal spray did not produce a significant reduction in vertebral fractures. Clearly, calcitonin produces a much weaker effect on bone than other agents for osteoporosis management. Bisphosphonates Bisphosphonates are synthetic compounds that were initially developed for industrial use, to inhibit the growth of crystals. They were soon discovered to have a number of biological effects, including the ability to inhibit osteoclast-mediated bone resorption. The magnitude of this effect depends on a number of factors, including inherent drug potency, dose, route of administration, and rate of bone turnover. Alendronate is the most potent drug currently available for the treatment of osteoporosis. The agent was studied in the Fracture Intervention Trial FIT ; , a very large, randomized, controlled trial that included 6, 459 participants and several years of observation Table 2 ; .18 All of the enrolled women had low hip bone mineral density scores T-scores -1.6 ; . Women were randomized to receive either 5 mg day of alendronate or placebo. On the basis of new data indicating improved efficacy on bone mass, the dose was increased to 10 mg after two years. The dropout rate was less than 4%. The women who received alendronate achieved substantial increases in bone mineral density mean, 7%-9% at the spine and 5%-8% at the hip compared to placebo ; . Alendronate was also shown in this trial to reduce the risk of hip fracture by 56% by year four in women who had osteoporosis but no prior vertebral fractures. The incidence of new vertebral fractures was reduced by 50%.18 Meta-analyses of all of the clinical trials of alendronate showed consistent reductions in hip-fracture risk of approximately 50%, as well as significant reductions in the risk of vertebral and wrist fractures.19.

The Commission on Mental Health and Community Solutions has worked extremely hard this past year to cover the wide spectrum of care and processes effecting Brevard County's mentally ill. The effort has been extensive and yet falls short of the impact they sought to achieve. The system of care has been found to be fragmented, inadequate, and the crisis has been found to extend beyond the jail and into the hospital emergency rooms. The need exists for the recommendations identified to be followed through to completion and further research and analysis conducted on identified solutions. The recommendations making the most significant impact are cost based. There will be no quick and easy solutions to the mental health crisis faced in Brevard, or in Florida. The system of care has remained constant without due compensation for growth expansion. Brevard County cannot neglect this issue and expect that it will somehow disappear. As our population grows, so grows our mental health needs. The impact of mental illness is being recognized nationally and statewide through the efforts of the President's Freedom Commission on Mental Health and continued advocating by the National Alliance for the Mentally Ill NAMI ; and Florida Partners In Crisis. The objective of our study was to determine if screening a normal population of women who might have an elevated risk of having a genetic mutation makes sense. We performed a series of cost-effectiveness analyses and found that if one only looks at survival, testing among a population of women such as those of Ashkenazi Jewish descent for genetic mutations is cost-effective. However, when one takes into account quality of life, we found that testing is beneficial only if the strategies to prevent breast and ovarian cancer are acceptable. The message from this study is that we need to develop better preventive treatments, such as tamoxifen or raloxifene to prevent breast cancers. More clinical research is needed before population screening can be recommended on a large scale unless women have 1st degree relatives who were diagnosed with breast or ovarian cancer at a young age. This past month we are starting work on a project to assess the value of MRI screening among women who have tested positive for these mutations. It will take close to one year to complete. We have been making good progress and are developing preference ratings among a group of women who have tested positive for mutations and are undergoing yearly MRIs and mammograms in Toronto.
