Synopsis The use of optimised medical treatment in elderly patients with chronic angina offers comparable results to invasive interventions with regards to 1-year outcomes, according to article published in the March 5 issue of the Journal of the American Medical Association JAMA 2003; 289: 1117-1123 ; . Researchers assessed the long-term value of invasive vs. medical management in 282 patients aged over 75 years with Canadian Cardiac Society class 2 or higher angina despite treatment with 2 or more anti-anginal drugs, who were enrolled in the Trial of Invasive versus Medical therapy in Elderly patients TIME ; study. The mean age of subjects was 80 years and all patients had to have an expected survival of at least 6 months post enrolment in the study. The main outcome measures were quality of life, assessed by standardized questionnaire; major adverse cardiac events death, nonfatal myocardial infarction, or hospitalisation for acute coronary syndrome ; after 1 year. A total of 140 patients were randomised to undergo coronary angiography followed by revascularisation if feasible ; and the remaining 142 patients received optimised medical therapy. Evaluation of patients at one year found that improvements in angina and quality of life persisted for both therapies compared with baseline, but the early difference favouring invasive therapy had disappeared by one year.No significant differences were observed between the two groups for 1-year mortality 11.1% for invasive; 8.1% for medical; HR, 1.51; 95% CI, 0.72-3.16; P .28 ; or death and nonfatal myocardial infarction rates 17.0% for invasive; 19.6% for medical; HR, 0.90; 95% CI, 0.53-1.53; P 0.71 ; .
