Objective: It has been suggested that dietary phytoestrogen intake during adolescence may reduce the risk of developing breast cancer. This population-based case-control study evaluated the association between adolescent dietary phytoestrogen intake and adult breast cancer risk among women in Ontario, Canada. Methods: Pathologyconfirmed, population-based breast cancer cases, aged 25-74 years, diagnosed between June 2002 and April 2003, were identified using the Ontario Cancer Registry. Population-based controls were recruited, and matched to cases within 5-year age groups. Adolescent phytoestrogen intake was obtained using a brief food frequency questionnaire n 3, 024 cases, n 3, 420 controls ; . Logistic regression was used to estimate odds ratios ORs ; and 95% confidence intervals CIs ; . Results: Higher phytoestrogen intake both isoflavones and lignans ; during adolescence was associated with a reduced breast cancer risk, and a monotonic trend was observed from the lowest to the highest quartile OR [Q2] 0.91, 95% CI 0.79-1.04, OR[Q3] 0.86, 95% CI 0.75-0.98, and OR[Q4] 0.71, 95% CI 0.62-0.82, p-trend 0.001 ; . Conclusion: Adolescent dietary phytoestrogen intake may be associated with a decreased risk of adult breast cancer. If verified, this finding has important implications with regard to breast cancer prevention since diet is a potentially modifiable factor.

Isoflavones are a group of diphenolic secondary metabolites produced in a very limited distribution of higher plants, most frequently in the Leguminosae 1 ; . Among the common dietary legumes, soybean contains the highest level of isoflavones. From the natural food sources, soybeans and soy foods contain the most dietary isoflavones, which provide many health benefits. The major types of isoflavones in soybean are daidzein, genistein, and glycitein. Most of these isoflavones in soybean seeds are conjugated with glucose or malonylglucose Figure 1 ; . In addition, acetylglucose conjugates are also detected in small amounts in soy products, but it appears that acetyldaidzin and acetylgenistin may be generated from daidzin and genistin during their heat-induced decompositions 2 ; . There are many reports that soybean isoflavones may have potential benefits for reducing the occurrences of diseases afflicting humans, such as certain types of breast, prostate, and colon cancer 3, 4 ; . Besides being anticarcinogenic, they exhibit antiatherosclerotic, blood glucose lowering, antibacterial 5 ; , and antioxidative 6 ; properties. With the discovery of increased cancer risks associated with estrogen-based hormone replacement therapy, the use of isoflavones as an alternative for menopausal women has received much public and scientific interest 7, 8 ; . In addition, isoflavones may reduce low-density lipoproteins and increase high-density lipoproteins, which help and isoniazid.

Smart MD Texas Heart Institute, Baylor College of Medicine, Houston, TX PURPOSE: pproximately one third to one half of patients with heart failure have renal insufficiency, which is one of the strongest predictors of mortality in these patients. However, there is little evidence with which to weigh the risks of cardiac transplantation in heart failure patients who have mild renal dysfunction. METHODS: We reviewed the clinical and laboratory data of 140 consecutive, stable heart failure patients who had mild renal insufficiency serum creatinine, 1.1-2.5 ; referred to our institution for evaluation for cardiac transplantation from May 2004 to April 2005. We then assessed the association between renal dysfunction and 1-year outcomes either death from any cause or admission for heart failure ; . The combined endpoint was compared with various levels of baseline serum creatinine values: mild 1.1-1.5 ; , moderate 1.6-2.0 ; , and severe 2.1-2.5 ; . Of the 140 patients, 98 had follow-up visits where laboratory values were obtained. In this subgroup, logistic regression analysis was performed to the model hazard ratio HR ; to achieve the combined endpoint. RESULTS: The cohort was predominantly Caucasian 45% ; and male 68% ; . After 1-year, the combined endpoint was met in 43%, 57%, and 42% of patients with mild, moderate, and severe elevations of serum creatinine, respectively. There was no statistical difference between the 3 groups Kruskal-Wallis One-Way ANOVA p 0.36 ; . However, in the 98 patients who had follow-up laboratory testing during the study period, a dynamic change in serum creatinine levels was strongly associated with a combined outcome HR, 1.70; 95%CI, 1.22-24.6; P 0.02 ; . This association remained significant even after adjustment for other clinical variables, including patient age and baseline serum creatinine levels HR, 1.71; 95%CI, 1.18-26.1; P 0.02 ; . CONCLUSION: A dynamic change in serum creatinine values during follow-up is a strong independent predictor of a worse prognosis for heart failure patients. CLINICAL IMPLICATIONS: These findings suggest that monitoring of serum creatinine values may offer a readily accessible tool to identify which heart failure patients would or would not benefit from cardiac transplantation. These results may indicate that, in animals, CatOMT activity, which occurs in different tissues 20, 21 ; , participate in the metabolization of plant products ingested in food. Thus the CoA moiety offers the possibility of additional interactions with the protein. Biochemical studies using truncated caffeoyl-CoA as putative substrate for CCoAOMT demonstrated the crucial role of 5 -phosphoadenosine moiety in enzyme binding. Furthermore, modeling of caffeoyl-CoA in the active site revealed several putative binding sites for the CoA moiety. Mutational analysis of the amino acid residues presumed to participate in the substrate binding showed the importance of several of them for catalytic activity. Arg-220 is probably involved in electrostatic interactions with the negatively charged phosphodiester bond in the CoA ester molecule while Asp-58 and Gln-61 are probably involved in binding of the adenine base. However, whereas substitution of Arg-220 totally eliminated enzymatic activity, the mutations of Asp-58 and Gln-61 change the kinetic properties of the enzyme, which was still active although less efficient Fig. 6 ; . Our data indicate an essential role of the coenzyme A moiety for caffeoyl-CoA recognition by CCoAOMT and catalytic activity, thus explaining the strict specificity of the enzyme for hydroxycinnamoyl-CoA esters 18 ; . The recent elucidation of the structures of chalcone and isoflavone OMTs has shown that, in another group of plant OMTs, protein dimerization plays an important role in substrate discrimination since the dimer interface contributes to the substrate binding site 19 ; . Similarly, the homo- or heterodimerization of OMT isoforms involved in alkaloid berberine biosynthesis has been shown to give rise to enzymes accepting new substrates.

The incidence of "false positive" ELISAs ranges from 5% in patients on hemodialysis, to ~10% of medical and surgical patients receiving heparin, 35, 36 to approximately half of all patients with recent heparin exposure after cardiac bypass surgery.5, 25, 37 Thus, 25 times as many patients develop anti-PF4 heparin antibodies after CABG as develop HIT T.38 These findings have several implications.
