Preview : five new antipsychotic drugs introduced in the united states in the last decade offer physicians the ability to treat patients with schizophrenia and bipolar mania without the adverse effects of the first-generation antipsychotics. In May 2002, the National Institute for Health Care Management, in a report titled, Changing Patterns of Pharmaceutical Innovation, suggested that many new drugs are really "me-too" drugs with little advantage over existing medications. This conclusion refutes the drug industry's claims that high prices are needed for research into important and innovative new drugs. Incubations with cDNA-expressed enzymes Assays were conducted in 96-well microplates. Aromatase enzyme baculovirus insect cellexpressed ; and the substrate DBF was obtained from BD Biosciences Discovery Labware Woburn, MA ; . Bifonazole, clotrimazole, econazole, ronidazole, tinidazole, thiabendazole, sulconazole and 1-benzylimidazole were obtained from Sigma-Aldrich. Sulfaphenazole, fluconazole, tetraconazole and itraconazole were obtained from Ultrafine Woburn, MA ; , ICN Biomedicals Aurora, OH ; , Crescent Chemicals Islandia, NY ; and Research Diagnostics Inc Flanders, NJ ; , respectively. A 50% inhibitory concentration IC50 ; was determined utilizing 12 wells in each test. A cofactor serial dilution C SD ; buffer contained an NADPH-regenerating system and 0.1 mg mL microsomal protein prepared from insect cells infected with wildtype virus "control protein" ; . To the first well in each row, 150 l of the test compound [dissolved in acetonitrile, or, DMSO itraconazole only ; ] and C SD buffer were added. Compound was diluted serially 1: 3 through the 8th well. Wells 9 and 10 contained no inhibitor and wells 11 and 12 were used as controls for background fluorescence [enzyme substrate E S ; added after the reaction was terminated]. Plate was then warmed to 37C and reaction initiated by the addition of E S. The E S mix contained buffer, P450 enzyme 2 pmol mL final ; , control protein 0.25 mg mL final ; , substrate 0.2 M final ; . Reactions were terminated after 30 min by addition of 75 l NaOH. Fluorescence signal was measured ex 485, em 538 ; using a FLUOstar model 403 fluorescence plate reader BMG LabTechnologies, Inc., Durham, NC ; . The IC50 values were calculated by linear interpolation and diamicron. Cell solutes leading to cell death and better drug penetration and antimicrobial activity. Note - since EDTA stimulates the release of LPS from the outer membrane it is less effective at inhibiting gram-positive than gram-negative bacteria because in gram-positive bacteria lack an outer membrane, which contains lipopolysaccharides. Bacteria have an efflux pump that is mediated by the MEX gene. This protein pumps the drugs out the bacteria, rendering the antibiotic ineffective. EDTA blocks this pump thereby allowing antibiotics to accumulate in the bacteria. To maximize its bactericidal activity it is essential for EDTA to be in environment w an alkaline pH. Appropriate pH 8.0 ; is maintained by combining it with buffers such as tromethamine TRIS ; hydrochloride. This alkaline pH also decreases the bacterial MIC for aminoglycocides and fluoroquinolones. It is therefore useful to use TrisEDTA prior to instilling either of these antibiotics. Two commercial veterinary preparations are available - T8 Solution, DVM Pharmaceuticals and TrizEDTA, DermaPet. The ear canal should be filled with the solution 15-30 min before instilling the topical antibiotic. This is done q 8-12 hrs. EDTA is used primarily for treatment of otitis externa caused by gram-negative organisms especially Pseudomonas. There is now a product that contains EDTA with 0.1% ketoconazole called T8 + keto DVM Pharmaceuticals, Inc. ; . In a study, sponsored by the company, they compared the effectiveness of T8 + ketoTM, T8 alone TrisEDTA ; and purified water against Malassezia. They mixed a solution containing Malassezia pachydermatis with each of the test products and then culture the mixture every 15 minutes for 2 hours. The results revealed only T8 + keto had any effect on the Malassezia pachydermatis. Significant reduction occurred w in 15 minutes of contact. I think this product will be helpful in the cases of a mixed rod and yeast infection. Regardless, one way or the other, all cases of Pseudomonas otitis media cases and externa for that matter ; will have Tris EDTA as a component of the therapy, either as a mixture w other agents or as a pretreatment followed by antibiotics. In the case of Pseudomonas ear infections otitis externa and or media ; I will use the following combinations- remember to find the underlying cause ; gentamicin or polymyxin B or E, or amikacin injectable more effective than tobramycin ; diluted from 250 mg ml to 50 mg ml and then use 1-2 cc bid or enrofloxacin 2800 mg of enrofloxacin use large animal injectable enrofloxacin 100 mg ml ; and mix w 8 oz TrisEDTA and flood the ear bid ; depending on what antibiotic has been used in the past. An alternative to the enrofloxacin EDTA mixture is to use synotic w 3 cc enrofloxacin added 10-15 drops tid and Triz EDTA hr before the synotic enrofloxacin. I will use silver sulfadiazine mixed 1: w water ; sid concurrently. When using both EDTA and silver sulfadiazine don't apply them w in 1 each other because of the potential that EDTA may decrease silver sulfadiazine's effectiveness by chelating the silver. Please note that necrotic tissue will decrease the effectiveness of silver sulfadiazine or aminoglycocides so only use these products in a clean ear. If Malassezia is present, an otic product containing gentamicin, betamethasone and clotrimazole may be substituted for the synotic enrofloxacin mixture. With any of the above treatment, I will use Synotic as part of the therapy unless one of the product that are being used has steroids in it The number one reason that I see a resistant pseudomonas infection is the failure to insist on rechecks and the casual dispensing of gentamicin or enrofloxacin containing topical medications. It is bad medicine and bad business to dispense medications without re ; evaluation of the ear. Systemic antibiotics It is important to appreciate that antibiotics, including the fluoroquinolones, can't exceed MIC levels w systemic administration while high drug concentration has been documented in the middle when treated with topical antibiotic in humans. In humans it has been suggested that S. aureus and P. aeruginosa are thought to cause sufficient tissue destruction to impede delivery of systemically administered agents. It is also difficult to reach effective concentrations w systemic drugs due to the limited blood supply to the TC. If the infection fails to respond to topical therapy and flushing, the addition of systemic antibiotics MAY be useful especially in the presence of a resistant Pseudomonas infection. Systemic therapy should be reserved for cases that fail to respond to topical therapy including bullae lavage and installation of medication. Topical therapy presumably fails because antibiotics cannot reach infected tissues. Systemic therapy is expected to succeed in penetration of the tissues. Many times the only effective antibiotics are the following: please note that I have only used the drugs w * next to them so please become more familiar w the other antibiotics before using 1. L Borgia, 1 SJ Valberg1, M MCue1, J Pagan2 and C Roe3 1College of Veterinary Medicine, University of Minnesota, St.Paul, MN, 2Kentucky Equine Research, Versailles KY, 3Baylor University, Dallas TX. Horses with polysaccharide storage myopathy PSSM ; develop exertional rhabdomyolysis ER ; and a deficit in energy generation during short-term aerobic exercise. We hypothesized that repeated rhabdomyolysis may result in loss of citric acid cycle CAC ; intermediates further disrupting energy generation. Repletion of CAC intermediates could be accomplished by provision of a synthetic odd carbon fat called triheptanoin C7 ; that would replenish CAC cycle intermediates by generating both acetyl CoA C2 ; and propionylCoA C3 ; , which enters the CAC as succinyl-CoA. Traditional management of PSSM involves decreasing starch intake and provision of even chain fat primarily linoleic C18 ; that undergo beta oxidation entering the CAC as C2. A low starch, fat enriched diet decreases ER in PSSM horses as indicated by lower serum creatine kinase activity CK ; . The purpose of this study was to determine whether feeding C7 as a source of dietary fat to PSSM horses would decrease serum CK activity following submaximal exercise more effectively than corn oil C18 ; . Initially, we gave fasted PSSM horses 0.5 gm kg of triheptanoin via nasogastric tube and demonstrated peak plasma C7 [acylcarnitine] occurred 90 min and peak plasma C5 and C3 [acylcarnitine] occurred 120 min after administration. A randomized cross-over design using 8 PSSM horses fed an isocaloric diet consisting of ration balancer, hay cubes and grass hay and either 1.5 mls kg day of C7 or C18 was used. An individual exercise regime was first established for each horse while horses were fed 0.05 MCal kg day diet consisting of sweet feed rice bran and grass hay. Horses exercised 5 days per week for 3 weeks on each diet with 1 week wash out period between diets. Blood samples were obtained 4 h after exercise for measurement of serum CK activity and acylcarnitines. Muscle biopsies were obtained pre and post exercise on the last day of each diet. All horses completed their daily exercise and diclofenac.

That augmentation of endothelial IKCa1 expression may be related to an activation of the endothelium by tumor-derived angiogenic factors and thus EC proliferation and tumor angiogenesis. In the light of this previous finding, we therefore tested the hypothesis that angiogenic factors modulate IKCa1 expression and function in human ECs and that selective blockade of IKCa1 channels inhibits EC proliferation. We demonstrate that the angiogenic factors VEGF and bFGF induced upregulation of functional IKCa1 expression in human umbilical vein ECs HUVECs ; and human microvascular ECs 1 HMEC-1 ; . Inhibition of the channel by the IKCa1 blocker coltrimazole CLT ; 11 and by the highly selective CLT derivative 1-[ 2-chlorophenyl ; diphenylmethyl]-1Hpyrazole TRAM-34 ; 12 that does not block cytochrome P450 enzymes12 abolished bFGF- or VEGF-induced cell proliferation in vitro and vascularization of Matrigel plugs in vivo. This suggests that IKCa1 controls EC proliferation and angiogenesis, and selective blockade of endothelial IKCa1 may represent a new therapeutic strategy to prevent tumor angiogenesis and ditropan.

Agents such as thapsigargin, which induce a rise in pulmonary vascular resistance. Previously, Rippe et al. 35 ; and Townsley and colleagues 47, 48 ; found that a marked vasoconstriction induced by various agonists such as norepinephrine, histamine, serotonin, and AA does not influence Kf, c. This point is further validated by the fact that lung lobes from paced animals have an elevated vasoconstrictor response to thapsigargin compared with control lobes, but the drug still fails to modify permeability 21 ; . Finally, one caveat to our conclusion that endotheliumderived cytochrome P-450 metabolites of AA are involved in thapsigargin-mediated increases in endothelial permeability is that our results could be indirect due to the effects of these drugs on cells other than endothelium. Thapsigargin is known to stimulate Ca2 influx in neutrophils 26 ; , and imidazole antimycotics such as clotrimazole have been shown to decrease agonist- or thapsigargin-stimulated Ca2 influx in both neutrophils and platelets 26, 38 ; . In addition, platelets at least are known to synthesize both EETs and 20HETE from AA 52 ; , although only 20-HETE has been reported from neutrophils 20 ; . Thus, in our bloodperfused lobes, it is possible that thapsigargin stimulates release of secretory products such as plateletactivating factor or oxygen radicals from either platelets or neutrophils and that inhibition of such by cytochrome P-450 inhibitors could potentially explain our results. Although possible, we believe that this scenario is unlikely. For example, clotrimazole has been shown to inhibit phorbol ester-induced release of superoxide anions from neutrophils 19 ; . However, this effect required doses of clotrimazole severalfold higher than that used in the present study. Furthermore, pretreatment with oxygen radical scavengers does not prevent thapsigargin-induced increases in microvascular permeability in buffer-perfused rat lungs 7 ; , a model where neutrophils at least are known to be retained in the lung. In addition, platelet-activating factor, a secretory product of neutrophils and platelets, does not increase endothelial permeability in blood-perfused canine lung 41 ; . Finally, in contrast to the stimulatory effect of 5, 6-EET on Ca2 influx in endothelial cells 17 ; , EETs have been shown to inhibit thapsigargin-induced entry of extracellular Ca2 in platelets 24 ; . We are not aware of any study in which the effect of EETs on neutrophil function has been evaluated. Collectively, these observations argue against an indirect role for cytochrome P-450-mediated signaling in the thapsigargin permeability response in the canine lung. Nonetheless, our conclusions regarding these results are suggestive rather than definitive. In summary, results of this study show that the epoxygenase metabolite 5, 6-EET can increase pulmonary microvascular permeability in normal, bloodperfused canine lungs. Furthermore, inhibition of the formation of EETs with two distinct cytochrome P-450 inhibitors attenuates the marked rise in permeability induced by thapsigargin. Our data therefore suggest that metabolism of AA via cytochrome P-450 epoxygenases is involved in pathways mediating pulmonary endothelial permeability and may provide a signaling.
