It is best not to "mix apples with oranges." For example, if you have an Alert running on competitors in an industry-news database and another in a patent database on a technology of interest, it is probably best to leave them as separate Alerts. It is certainly possible to combine them, but there is no cost advantage to do so, and it could be very confusing to manage if you have the results pooled in a single Alert. Similarly, if you have two Alerts running that go to different individuals, then it may be best to keep them separate for ease in tracking. Many databases offer unique indexing that enables very precise retrieval; however, a very precise search strategy in one database may not work well in another. The use of different terminology and spelling, as well as the presence or absence of specific data elements, limits cross-file compatibility. This is especially true for searches involving the use of additional index fields or descriptor terms. For example, the major biomedical databases, MEDLINE, EMBASE and BIOSIS Previews, each uses a different controlled vocabulary term for "gestational diabetes" see "The FROM Qualifier" sidebar ; . If you already expended much effort crafting a model search to take advantage of a lot of databasespecific indexing, it is best to leave it running in just that database. HOW DO I COMBINE SEPARATE ALERTS INTO A "MULTIFILE" ALERT? There are several methods you can use to convert an existing Alert into a "multifile" Alert. They are: 1. Modify your current Alert using the EDIT SAVE ALERT commands DialogClassic or DialogWeb ; 2. Recreate the Alert using the EXECUTE command with an existing Alert 3. Modify your current Alert using the EDIT command 4. Create a new Alert; delete the old Alert Each method is described and illustrated in the online version of this story at : library chron 2002 0009. Note that every Alert on Dialog is a saved search SearchSave ; with a unique name. The Alert name begins with a Dialog-assigned two-character code, followed by either the name you enter to SAVE the Alert or a number assigned by the system. You can modify an Alert without incurring DialUnit charges by making your modifications while in a free database: Chronolog File 410 ; or Bluesheets File 415 ; . GETTING HELP Help in modifying or creating Alerts is available from Dialog online, by e-mail, and by telephone, for example, cefepime.

The following agents were employed: BAY 12-8039 and ciprofloxacin Bayer AG, Wuppertal, Germany ; , trovafloxacin Pfizer Inc., Groton, Conn. ; , cefpodoxime Roussel Uclaf, Romainville, France ; , amoxicillin and clavulanic acid SmithKline Beecham, Worthing, United Kingdom ; , rifampin Sigma, Poole, United Kingdom ; , rifampin Sigma, Poole, United Kingdom ; , and erythromycin Lilly Products, Basingstoke, United Kingdom ; . All agents were prepared and stored following the manufacturer's instructions. Susceptibility testing. A total of 684 recent clinical isolates, 11 control strains, and 10 well-characterized -lactamase-producing strains were studied. The control strains used were Escherichia coli NCTC 10418 and ATCC 25922, Pseudomonas aeruginosa NCTC 10662 and ATCC 27853, Staphylococcus aureus NCTC 6571 and ATCC 29213, Streptococcus pneumoniae NCTC 7465 and ATCC 49619, Haemophilus influenzae NCTC 11931 and ATCC 49247, and Enterococcus faecalis ATCC 29212. Susceptibilities were determined by a standard agar plate dilution method following recommendations in reference 1. Briefly, Iso-Sensitest agar pH 7.2; Unipath, Basingstoke, United Kingdom ; was employed for aerobic bacteria, supplemented with 50 g of 1- 4-nitrophenyl ; -glycerol BDH, Poole, United Kingdom ; per ml where necessary to prevent swarming. Supplements of 5% horse blood Bradsure Biologicals, Loughborough, United Kingdom ; and 20 g of NAD Sigma ; per ml were added to support growth of fastidious bacteria. For anaerobic bacteria, Wilkins-Chalgren agar Unipath ; supplemented with 50 g of 1- 4-nitrophenyl ; -glycerol per ml and 5% horse blood was used. All strains were tested at a final inoculum of 104 CFU and for a few selected strains at an increased inoculum of 106 CFU, using a multipoint inoculator Denley Instruments, Billingshurst, United Kingdom ; . Plates were incubated at 35 to for 18 to 24 air; or, for fastidious bacteria, in an atmosphere enriched with 4 to 6% carbon dioxide; or, for anaerobic bacteria, in an anaerobic cabinet Don Whitley, Shipley, United Kingdom ; in an atmosphere of 10% hydrogen, 10% carbon dioxide, and 80% nitrogen. The MIC was defined as the lowest antibiotic concentration at which no more than two colonies were observed. Amoxicillin and clavulanic acid were combined in a ratio of 2: 1, and the results were recorded in terms of the amoxicillin MIC. Mycobacterium susceptibility testing. The activity of BAY 12-8039 against mycobacteria was studied by an agar incorporation method using rifampin as a comparative agent. Recent clinical isolates of Mycobacterium tuberculosis three resistant to one or more of the commonly used antimycobacterial agents and one susceptible strain ; were studied. For both antibiotics a concentration range of 0.015 to 128 g ml doubling dilutions up or down from 1 g ml ; incorporated into Middlebrook 7H10 medium Difco, Detroit, Mich. ; , containing 10% Middlebrook oleic acid-albumin-dextrose-catalase enrichment as a supplement, was used. Plates were incubated at 37 C 10% carbon dioxide for 21 days. The lowest concentration of antibiotic that inhibited more than 99% of the bacterial population was considered to be the MIC 8 ; . Chlamydia susceptibility testing. The activity of BAY 12-8039 against one strain of Chlamydia pneumoniae and 3 strains of Chlamydia trachomatis was investigated in comparison with those of ciprofloxacin and erythromycin. The method employed was an adaptation of that of Webberley et al. 11 ; . The MIC was taken as the lowest concentration to inhibit the development of inclusion bodies, and the minimum lethal concentration MLC ; was defined by the absence of inclusion bodies after a further 48-h incubation in drug-free medium. Serum effect. The effect of human serum on the MIC and minimum bactericidal concentration MBC ; of BAY 12-8039 was determined for two strains each of Streptococcus pyogenes, S. pneumoniae, methicillin-sensitive S. aureus MSSA ; , Moraxella catarrhalis, E. coli, and Klebsiella pneumoniae. A microdilution method was employed using Iso-Sensitest broth Unipath ; containing 20 or 70% human serum Bradsure Biologicals ; and supplemented for fastidious bacteria with 5% lysed horse blood and 20 g of NAD per ml. Concentration ranges doubling dilutions up or down from 1 g ml ; BAY 12-8039 were 0.008 to 8 g 0.03 to 32 g for fastidious bacteria ; . A final inoculum of 105 CFU ml was used. Food is shaped by those who feed us in early years. It is believed that women with eating disorders may even inadvertently ; be teaching their children to follow in their footsteps. Eating habits during pregnancy may even hard-wire a baby in a way that makes him susceptible to a variety of problems, including eating problems, diabetes and depression. If you are pregnant and answer yes to any of the following questions, seek the guidance of a registered dietitian specializing in prenatal nutrition and eating disorders, to be sure you give your baby the best start possible toward healthy childhood and adulthood. A licensed mental health professional can also be helpful for developing healthy coping and parenting skills that give your baby the best nutritionaland emotionalfoundation possible and propulsid. The food you eat should be tasty because it's important that the food you eat satisfies you. Use cooking methods that use minimal fat, such as baking, broiling, braising, poaching, sauteing, stir frying and microwaving. Try herbs and spices to enhance the flavor of your foods. Use small amounts of salt when you prepare foods and at the table. Satisfy your "sweet tooth" in healthy ways: with fruit, sorbet, low-fat yogurt, low-fat ice cream or angel food cake. Bacterial rhinosinusitis and AOM caused by H influenzae as well as 50% to 75% of the children infected with M catarrhalis also will recover spontaneously.45 Furthermore, only S pneumoniae that are highly resistant to penicillin will not respond to conventional doses of amoxicillin. Therefore, approximately 80% of children with acute bacterial rhinosinusitis and AOM will respond to treatment with amoxicillin. However, risk factors for the presence of penicillin-resistant isolates in children include daycare attendance, recent antimicrobial treatment 90 days ; , and age younger than 2 years.46 Antimicrobial pharmacokinetics are important considerations for use against a pathogen. It is important to evaluate the in vitro activity of an antimicrobial against the pathogen in conjunction with achievable concentrations at the site of infection. For example, macrolides such as clarithromycin and azithromycin are concentrated in leukocytes and have higher concentrations in alveolar epithelial lining fluid tissues compared with plasma.47 Therefore, these agents are very active for pneumonia. In addition, -lactams and some macrolides work in a time-dependent manner, which means that the duration of time their concentration stays above the minimum inhibitory concentration at the site of infection is critical to their success. Based on these criteria, high-dose amoxicillin remains a potent agent against S pneumoniae. Other highly active -lactams effective against S pneumoniae include cefdinir, cefpodoxime, cefprozil, and cefuroxime.48 Ceftriaxone also is highly effective against S pneumoniae. Due to high rates of -lactamase production by H influenzae and M catarrhalis, clavulanate -lactamase inhibitor ; is required in addition if amoxicillin is used. However, 3 cephalosporins cefixime, ceftibuten, and ceftriaxone ; have been shown to be highly effective against these pathogens.48 An important practical consideration for administration of antimicrobials is patient compliance. This is specifically important in children since it may be difficult to ensure administration of antimicrobials if dosing is too frequent or if they are not palatable. Table 1 lists the palatability ratings for common antimicrobial suspensions used for ARIs. When overall taste ratings were adjusted for duration and dosing of the antimicrobial, cefdinir was the most palatable oral suspension among potent antipneumococcal -lactams.49 These data may be useful for making decisions about use of antimicrobials, particularly in children. It should be noted that ciprofloxacin should be avoided in children due to concerns about cartilage toxicity and clopidogrel and cefpodoxime.

While the current American Academy of Pediatrics treatment guidelines for otitis media suggest some physicians may elect observation with symptomatic treatment instead of antibiotic therapy, the aminopenicillins amoxicillin ; remain the first-line treatment for most children with otitis media.32 Amoxicillin clavulanate is recommended for more severe illness and cefdinir, cefpodoxime, and cefuroxime are recommended for children allergic to amoxicillin. Alternative recommended therapies include azithromycin, clarithromycin, erythromycin -sulfisoxazole, and sulfamethoxazole -trimethoprim. Clinical data presented suggests erythromycin sulfisoxazole and other antibiotics for otitis media are comparable in efficacy and safety. Additionally, generic options are available for many of the recommended first-line agents. Therefore, all brand products within the class reviewed are comparable to each other and to the generics and OTC products in that class and offer no significant clinical advantage over other alternatives in general use.
