Abstract. Rifaximin was compared with placebo and ciprofloxacin for treatment of travelers' diarrhea in a randomized, double-blind clinical trial. Adult travelers N 399 ; consulting travel clinics in Mexico, Guatemala, and India were randomized to receive rifaximin 200 mg three times a day, ciprofloxacin 500 mg two times a day and placebo once a day ; , or placebo three times a day for 3 days. Patients recorded in daily diaries the time and consistency of each stool and documented symptoms for 5 days after treatment. Stool samples were collected for microbiologic evaluations before and after treatment. The median time to last unformed stool TLUS ; in the rifaximin group 32.0 hours ; was less than one half that in the placebo group 65.5 hours; P 0.001; risk ratio 1.6; 95% confidence interval 1.2, 2.2; primary efficacy endpoint ; . The median TLUS in the ciprofloxacin group was 28.8 hours P 0.0003 versus placebo; P 0.35 versus rifaximin ; . Rifaximin was less effective than ciprofloxacin for invasive intestinal bacterial pathogens. Oral rifaximin is a safe and effective treatment of travelers' diarrhea caused by noninvasive pathogens. INTRODUCTION Travelers' diarrhea, usually contracted from contaminated food or water, affects 2050% of individuals traveling from developed to developing countries.13 Bacteria, most commonly enterotoxigenic Escherichia coli ETEC ; and enteroaggregative E. coli EAEC ; , are responsible for up to 80% of acute cases; other bacteria such as Shigella, Salmonella, and Campylobacter, as well as viruses and protozoa, make up the remainder of known causes.46 In most microbiologic studies of travelers' diarrhea, up to one third of the cases remain undiagnosed. Because many of these cases seem to respond to antibiotics and manifest indirect evidence of ETEC infection, such as seroconversion to the heat-labile enterotoxins, it has been assumed that ETEC or other undiagnosed bacteria were the cause.7, 8 Travelers' diarrhea can cause serious illness and be economically costly for travelers. One in three individuals with travelers' diarrhea requires bed rest, and one in five is confined to bed for 12 days.9, 10 Affected individuals may incur costs of medication, medical services, and canceled or rescheduled activities. In some cases, the impact of travelers' diarrhea extends beyond the individual sufferer. For example, among American soldiers in Saudi Arabia during Operation Desert Shield, mission preparedness was undoubtedly affected by the frequent occurrence of travelers' diarrhea, at least one episode of which was reported by 57% of 2, 022 soldiers surveyed in one study.11 Although travelers' diarrhea usually lasts less than 1 week, diarrhea can persist for more than 2 weeks in 210%, and as many as 10% can develop post-infectious irritable bowel syndrome.12, 13 Other sequelae of acute bacterial diarrhea include reactive arthritis with shigellosis and Guillain-Barr syndrome with Campylobacter infection.14, 15 The acute personal and economic impacts of travelers' diarrhea underscore the importance of effective treatment. The Infectious Diseases Society of America IDSA ; guidelines recommend empirical antibiotic therapy for travelers' diarrhea.16 However, current treatment options have significant shortcomings. 7, 17 The usefulness of trimethoprimsulfamethoxazole, one of the mainstays of antidiarrheal antibiotic therapy in the United States, is limited by the emergence of widespread bacterial resistance.1820 Ciprofloxacin, a fluoroquinolone antibiotic, is well tolerated and largely effective against E. coli and Shigella, but case reports of ciprofloxacin-resistant ETEC have been reported from travelers returning from India.21 More significantly, the emergence of ciprofloxacin-resistant Campylobacter species has decreased the effectiveness of ciprofloxacin for empirical treatment of travelers' diarrhea, particularly in Asia.22 In areas where Campylobacter is prevalent or quinolone treatment has failed, azithrojycin is now used as an alternative.23, 24 Azithromycin is also a systemic antibiotic that is used primarily for the treatment of respiratory infections. Although enteric pathogens are susceptible to azithromycin, the resolution of symptoms is not as rapid as with the fluoroquinolones.24 A non-absorbed antibiotic theoretically confers several advantages that have been identified as characteristics of the ideal antibiotic for infectious diarrhea7, 16, 17, 25: targeted activity at the source of infection, low potential for systemic side effects and drug interactions, and--because its value and use are limited to gastrointestinal infections--the potential for limited or delayed development of clinically relevant bacterial resistance. This randomized, double-blind, parallel-group study was conducted to compare the efficacy and tolerability of the non-absorbed oral antibiotic rifaximin Xifaxan, Salix Pharmaceuticals, Inc., Morrisville, NC ; with those of ciprofloxacin and placebo for the treatment of travelers' diarrhea. Rifaximin was first marketed in 1987 in Italy and is now approved in 17 countries, including most recently the United States, for treatment of gastrointestinal bacterial infections.

Proportion of material which is closely associated with the underlying geologicd formations. The original mantle of debris left by the glaciers has since been sorted by the action of streams and somewhat modified. Most of the area is covered with glacial till in the form of a ground moraine which varies in depth from a thin veneer to several feet. This till appears to be closely associated in minera1 composition with the underlying geologicl material and in some cases gives the appearance of being residual. It has been leached and weathered and its original texture, colour and composition have been changed to give the present-day soils. The colour of the till varies froln a light yellow, through grey to greenish grey and it is often compact. The appearance of the ground moraine reflects its glacial origin. For example, the area near Scotstown is strewn with granite boulders, presumably derived from the Megantic mountain mass to the eastward and the level area near Orford Centre, extending northward several miles, is dotted with small irregular Iakes, poorly-drained areas and covered with stony glacial till. On the higher ridges, the bed-rock is frequently exposed and in many places the outcrops are numerous enough to render the land unsuitable for cultivation. The glacial outwash deposits are scattered along the courses of the principal rivers, particularly along the St. Francis between Sherbrooke and Richmond. At Windsor Mills these deposits form large, conical or ridge-shaped hills having elevations of 50 to 100 feet above the present flood plain. I n the vicinity of Danville and also along the main highway south of Lennoxville, the gravel is usually stratified and covered with a few feet of fine and coarse sand containing small quantities of gravel. There are also gravel deposits occurring in the forin of kames and eskers in which the gravel is poorly sorted. Grave1 deposits are found in many places throughout Stanstead county a t an elevation of several hundred feet above the present stream level. Along the Coaticook, St. Francis and several of the smaller rivers, the outwash is deposited on lacustrine Clay, which is often many feet deep. The deposits over the Clay Vary in depth from a thin layer to two or t.hree feet. The recent alluvial deposits are typified by the flat flood plains along Ihe river courses. Usually these flat areas, which are composed of fine and coarse sand to a depth of several feet, are quite large and suitable for agricultural purposes. Several deposits of lacustrine Clay are found throughout the area, one of which is located on the Experimental Station at Lennoxville. This is a deep, heavy Clay and is usually poorly drained.
PLIVA's US generics business continued its impressive sales growth trend and surpassed market performance, increasing by 38% to USD 63m. Strong sales growth was fueled by the addition of generic azithromycin to PLIVA's US product portfolio as well as solid growth of the overall portfolio. Overall sales in Western Europe WE ; were flat CER + 1% ; and amounted to USD 53m. Strong growth was recorded both in the UK and Spain, where sales were up 25% CER + 27% ; to USD 8m and 54% CER + 54% ; to USD 9m respectively. This performance was offset by the slowdown in Germany and strong sales decline in Italy. Sales in Spain grew strongly driven by the integration of Uso Racional. New legislation in Germany, which introduced lower reference prices, price reductions, compulsory rebates and a price moratorium through to 2008, took effect in May negatively affecting overall market performance. In line with this market performance, PLIVA's sales in Germany slightly declined by 2% CER -2% ; to USD 32m. Sales in Italy, however, depressed WE performance, declining 74% CER -74% ; to USD 2m primarily as the result of the correction of certain revenue and expense recognition principles in 2006 which followed the identification of certain weaknesses in internal control and accounting practices in PLIVA's Italian operations. The expiry of PLIVA's US azithromycin patent at the end of 2005 continued to have the largest impact on Group results. PLIVA recorded USD 30m less in royalty revenues as well as a sharp decline in bulk azithromycin sales, which in turn led to a 78% decrease in Pharma Chemicals division's sales to USD 7m. PLIVA's WE markets grew by 3% CER + 8% ; to USD 106m, primarily due to the lower Q2 performance of both Italy and Germany. Noteworthy contributions were recorded in the UK, up 27% CER + 33% ; to USD 15m, and Spain, up 23% CER + 28% ; to USD 15m. Despite a rather flat second quarter, sales in Germany were up 5% CER + 10% ; to USD 66m, while Italy posted a 58% CER -55% ; decrease to USD 5m. During the period, 29 new molecules were launched across PLIVA's key markets, bringing total new product sales from 2005 2006 launches to 13% of total generics sales. PLIVA also submitted a total of 46 different molecules for registration, with 39 submitted in CEE, 11 in WE and 3 in the US. With a sales level of USD 46m, the Pharma Chemicals division recorded a decline of 44%, in line with management expectations for this division's performance by the end of the year, while Royalties decreased to USD 12m. Non Core divisions recorded sales of USD 26m, a decrease of 12% over the prior period. Other Income was down 37% to USD 13m, following the inclusion of a number of nonrecurring items recorded in H1 2005, such as property sale income and the USD 5m payment from Barr related to the biosimilars partnership on G-CSF.
Study Design and Source of Organisms Eight centers participated in this prospective multicenter in vitro study Figure 1 ; . During the sampling period October 2000 to March 2001 each laboratory was requested to randomly collect up to 70 fresh clinical isolates: 30 isolates each of S. pneumoniae and H. influenzae, and 10 isolates of M. catarrhalis. Copy strains were not included. Identification of Organisms At first, all isolates were identified locally and subsequently re-identified by the reference laboratory.
