Abstract: In contrast to classical b-adrenoreceptor antagonists, nebivolol and carvedilol possess endothelium-dependent vasorelaxant properties. It has been proposed that nebivolol and carvedilol activate microvascular endothelium into producing NO by the release of extracellular ATP and subsequent stimulation of endothelial P receptors. Here we tested this hypothesis in the coronary circulation of the isolated guinea pig heart. We analyzed the role of NO in the coronary vasodilatation induced by nebivolol and carvedilol as well as a possible involvement of extracellular ATP in these responses. Nebivolol and carvedilol 330 10`$ M ; induced a concentration-dependent coronary vasodilatation that was inhibited by NO-synthase inhibitor, L-NAME 10`" M ; . In contrast to nebivolol and carvedilol, neither atenolol nor labetalol acted as a coronary vasodilator. Vasodilatation induced by nebivolol and carvedilol was affected neither by the P receptor antagonist, 8-sulfophenyl theophylline 8-SPT, 10`# M ; , nor by the P receptor antagonist, suramin 10`# M ; . On the other hand, ATP-induced coronary vasodilatation 0.310 10`$ M ; was strongly inhibited by L-NAME 10`" M ; , partially inhibited by 8-SPT 10`# M ; , while suramin 10`# M ; had a minor effect. In conclusion, in the isolated guinea pig heart nebivolol and carvedilol, but not their classical counterparts atenolol, labelatol ; , act as NO-dependent coronary vasodilators. The anti-hypertensive and anti-anginal effects of nifedipine GITS have been well established for more than 15 years. Nifedipine GITS has demonstrated a reduction of elevated blood pressure effectively in patients with mild to moderate hypertension in placebo-controlled and non-comparative clinical trials. Response rates were higher in elderly patients than in younger patients.9, 10 No differences in blood pressure-lowering effects were observed with regards to gender, ethnic origin, body weight, and presence of diabetes. In a number of comparative trials with nifedipine GITS, compared with other drug classes, investigators have shown equal or superior anti-hypertensive efficacy of nifedipine GITS. These studies included calcium channel blockers verapamil, diltiazem and felodipine ; , betablockers atenolol, propranolol ; , and angiotensinconverting enzyme ACE ; inhibitors enalapril and lisinopril ; .9, 10 In comparison with co-amilozide hydrochlorothiazide amiloride ; , nifedipine GITS was demonstrated to be the equivalent in effectively lowering 24-hour blood pressure.9 Following the development of nifedipine GITS, studies demonstrated that patients could be switched from alternative formulations of nifedipine with comparable or improved efficacy, with a resulting improvement in the side effects profile.11 Subsequent studies suggested that nifedipine GITS had equivalent efficacy to diltiazem and atenolol in increasing exercise tolerance in chronic stable angina patients.1214 Studies also suggested that the efficacy and tolerability of nifedipine GITS was superior to long-acting nitrates as second-line therapy to betablockade in the treatment of chronic stable angina15 and was particularly effective in patients with chronic stable angina inadequately controlled on betablockers alone.16, 17 Nifedipine GITS has also been shown to have a sustained effect a single daily dose was effective over 24 hours regardless of whether it was administered in the morning or evening.16 In addition to being efficacious and safe, nifedipine GITS is apparently able to maintain quality of life in patients with mild to moderate hypertension.9.

An important study limitation is that the survey was sent only to those participants who were still taking study pills when the intervention was stopped. More than 40% of trial participants had already discontinued study pill use for personal or protocol-related reasons and were not included in this survey. These excluded women may have had different symptom experiences before, during, and after discontinuing study pill use compared with the survey respondents who took study pills for a longer period. Although there were no significant differences in the proportions of survey-ineligible participants and respondents who had moderate or severe hot flashes at baseline, a significantly greater proportion of survey-ineligible participants than respondents did have moderate or severe night sweats at baseline. It may be that night sweats, more than hot flashes, were disruptive to participants' daily lives during follow-up and therefore prompted more women with these symptoms to discontinue study pill use before the stop date. During followup, survey-ineligible participants, particularly those assigned to active CEE MPA, more often identified symptoms associated with MHT eg, breast changes and vaginal bleeding ; rather than menopausal symptoms as reasons for discontinuing study pill use. Although vasomotor symptoms and vaginal changes were given as reasons for discontinuing study pill use early by similarly low numbers of both CEE MPA and placebo participants, the experience of stopping study pills in these survey-ineligible participants may have been different from the experience of survey respondents. Participants who enrolled in the E P trial and continued taking study pills until July 2002 undoubtedly do not represent the full spectrum of symptom severity. The WHI clinical trial protocol.
3.2.8 Affordability of Treatment Regimens for Selected Common Conditions: Public, Mission and Private Sectors Whereas the treatment affordability for hypertension using the innovator brands of either Nifedipine 'Retard', 10 Captopril or Atenolol was about the same at 9 10 days' wages for the lowest paid government worker Fig. 3.21 ; , using the generic equivalents reduces the treatment costs of Nifedipine Retard to 1.8 days' wages, and Atenolol to 1.1 days' wages, whereas treatment with Captopril is relatively higher at 4.3 days' wages using the LPG.
